Mobile Crisis Case Manager
POSITION SUMMARY:
The South Central Crisis Service provides after-hours telephonic and mobile crisis response to persons in behavioral health crisis in the community with the goal of stabilizing in and remaining in the community without a need for hospitalization or incarceration.
This is a Per Diem after-hours and weekend mobile crisis case management position to assist in co-response with licensed clinicians to people in behavioral health crisis in the community.
The Case Manager will assist with connecting people in need with follow up services.
This position requires driving to the location for psychiatric assessment.
This is not an exhaustive listing of position duties, essential functions include:
Provide mobile crisis services for all hours committed to on the monthly schedule;
Fully complete all on-call clinical documentation within 2 hours of the mobile visit;
Assist clients with connecting to medical and/or psychiatric appointments as follow-up to crisis intervention.
Develop individualized and generalized community supports.
Provide education, support and consultation to family members, significant others and community members to support client stabilization following crisis intervention.
Collaborate and build relationships with local police departments.
Attend training of approximately 2-4 hours prior to performing the duties of crisis service clinician;
Consistent attendance at regular monthly after hours programmatic meetings;
Attend the SCCS Annual Training in June from 9am-4pm;
Complete monthly on-call schedule in monthly staff meeting or email and, if absent for meeting, will submit request for preferred dates to SCCS Clinical Coordinator prior to the staff meeting.
Inform After Hours Program Supervisor when unable to perform scheduled duties and make every effort to arrange for coverage by another case manager
Inform After Hours Program Supervisor of all HIPAA violations immediately;
Review and follow all CommuniCare and SCCS policies and procedures; Contact the Clinical Coordinator as outlined in the SCCS Policy and Procedure Manual.
Required to use own vehicle on company business.
Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
Regular and consistent attendance is an essential function of this position.
Perform any other department or agency related duties or special projects as directed.

Qualifications
Bachelor’s degree preferred.
In lieu of Bachelor’s degree: Associates degree or college coursework equivalent (2 years) and 2 years work experience in behavioral health field; OR high school diploma and minimum of 4 years of experience in behavioral health field.
Working knowledge of behavioral health recovery.
Must have and maintain a valid driver’s license.
Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
Must be proficient in Microsoft 365.
Must be able to navigate through electronic health records system (EHR).
