_*Must be licensed in NY and/or CT*_ *Job Description: Telehealth Advanced Practice Registered Nurse (APRN)/Nurse Practitioner* *Full Time* *Reports to:* Regional Director of Population Health with clinical supervision by Medical Director *Scope:* Performs remote video visits for patients.
Primarily a remote position with occasional in person requirements – meetings, testing, potential for in person visits within designated geography.
*Administrative duties/program development:* Participate in development of RPM/CCM and other clinical programs, development of clinical pathways and protocols, documentation templates and other tools, collaborate re: content of policies and procedures.
*Visits:* Initial visit will include consent for care, regardless of patient program.
Initial onboarding visits for RPM/CCM program, follow up visits and change in condition visits.
Home Health transitional care, follow up and change in condition visits.
Hospice: face to face visits, other follow up visits.
*Oversight of Plan of Care:* RPM/CCM – creation/approval of plan of care upon patient enrollment, and minimum monthly review of plan with additional review/updates as needed with changes in condition.
Oversight/review of readings and other staff interaction notes at time of review, minimum monthly.
*Collaboration with PCP and other care providers:* After each patient interaction, communication with MD via clinical note.
If patient is active on Home Health or Hospice, communication with clinical team re: findings/outcome of visit via clinical notes.
Phone calls will be made in addition to notes for urgent, time sensitive issues.
Practitioner will be available to attend Home Health or Hospice IDT meetings to report on/consult regarding patients seen, high risk patients or patients with changes in condition.
*Clinical Practice:* Must be proficient in industry best practices for diagnoses treated.
Protocols and tools developed will incorporate best practices.
Every visit will include an assessment of the patient status, any changes will indicate reassessment of level of support and proper level of care.
Practitioner will be proficient in knowledge of eligibility for all levels of care including Home Health, Hospice, Palliative and SNF.
Timely communication and resolution of issues identified on visits including communication with PCP, clinical team and patient/family.
Priority focus of visits will be to ensure proper level of care, patient safety and avoidance of unnecessary hospitalizations.
Practitioner will leverage known home diagnostic and treatment capabilities to advocate for patients who can safely be managed at home and avoid avoidable visits to Emergency Department.
Target for patient population managed ACH <=10% (can adjust this).
Plan of care and follow up visits will include a review of Advanced Directives and practitioner will initiate discussions around Goals of Care in collaboration with patient, family, PCP, other clinicians involved in the patient’s care.
*Documentation* Record timely (same day or within 24 hours) and accurate documentation of all encounters in the appropriate RPM/CCM platform and EHR.
*Training and oversight of other Telehealth APRNs* Train and ensure compliance with protocols by any additional APRN who are hired to support coverage/overflow visits.

Hearing and speaking ability is required to communicate on the telephone.

Reaching is required for paper management.

Manual dexterity is required to input computer information , entering data, operating the laptop and telephone.
5.
Mobility is required for occasionally attending meetings in person.
6.
Must be able to speak and write the English language in an understandable manner 7.
Must be able to function independently and have personal integrity.

Ability to travel to various office/sites 9.
May assume on call responsibilities as directed.
Work environment is characteristic of any office setting which involves prolonged periods of sitting, and intermittent physical activity such as occasional requirement to stand, walk, reach with arms and hands PSYCHOLOGICAL REQUIREMENTS 1.

Job Type: Full-time Pay: $97,120.
39 - $116,962.
