*Position Overview:*
As a licensed professional provider working collaboratively with attending physicians, you'll play a crucial role in providing high-quality, patient-centered care in various practice settings within YNHHS.
Joining a multidisciplinary team, you'll contribute to creating a patient-centric environment that addresses the physical, psychosocial, spiritual, and functional needs of patients, their families, and significant others.
*This position entails 1-2 days of operative time per week, specializing in breast surgery oncology.
Additionally, there are 2 days dedicated to clinic responsibilities.
Clinic locations encompass a range of sites, including but not limited to Guilford, North Haven, and New Haven.
The operational model is characterized by a collaborative and independent framework.

*Responsibilities:*
* *Clinical Care:*
* Assess, triage, and manage patient/family needs across different practice settings.
* Collaborate with supervising physicians, adhering to state laws, hospital regulations, and clinical pathways.
* Conduct medical histories, physical examinations, and formulate/manage treatment plans.
* Perform procedures and prescribe treatment within institutional and state guidelines.
* *Patient Education & Psychosocial Support:*
* Educate patients/families on care, treatment plans, and available resources.
* Provide psychosocial support in collaboration with the multidisciplinary team.
* *Information Management:*
* Ensure accurate documentation following hospital and regulatory requirements.
* *Quality Management:*
* Demonstrate high moral and ethical standards, professionalism, and commitment to patient care.
* Ensure compliance with healthcare regulatory requirements, standards, and recommendations.
* *Professional Development:*
* Engage in projects to maintain and advance professional competency.
* Serve as a clinical resource, mentor, and preceptor for healthcare professionals and students.
* Collaborate on research, quality assurance, and performance improvement projects.
*Qualifications:*
* *Education:* Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
* *Experience:* Clinical working experience within the specialty is preferred.
* *Licensure:* Active PA Certification (NCCPA) or Connecticut State Nurse Practitioner License.
*Additional Information:*
* Schedule: (4) 10-hour shifts per week, including clinic and operative time.
* Clinic sites: Guilford, North Haven, & New Haven.
* Surgical Oncology, breast surgery, and operative experience preferred.
